I hereby release to BLOOMFIELD COMMUNITY TELEVISION (BCTV) rights to my image likeness, the sound of my voice as recorded on videotape, and/or the use of my image likeness in Bloomfield Township print material. I understand that this video recording may be edited and cablecast, and thereafter the program may be otherwise available. For my participation in the videotaping or live programming sponsored by BCTV, I give permission to reuse all of my appearances on such videotaping or live programming.
I further agree to release, indemnify and hold Bloomfield Township and all of its BCTV personnel harmless from all legal or administrative proceedings, claims and damages arising from my failure to comply with the BCTV Access Policies & Procedures, or other actions or statements by me during my participation in the BCTV program(s) identified below.
I hereby release to BCTV all rights to the use of my image, likeness and sound of my voice if recorded as part of my participation in the program identified below, and give permission for BCTV to reuse all of my appearances on that program on any form of media sanctioned by the Township.
