Passport Services

On June 1, 2009, the U.S. government implemented the full requirements of the land and sea phase of the Western Hemisphere Travel Initiative (WHTI). The proposed rules require most U.S. citizens entering the United States at sea or land ports of entry to have a passport, passport card, or other travel documents approved by the Department of Homeland Security.

The Intelligence Reform and Terrorism Prevention Act of 2004 requires travelers have passports to countries that didn’t require them before.

As of February 1, 2008, the U.S. Dept. of State changed the fees, forms, and requirements and approved issuance of a new Passport Card.

In the fiscal year 2008, the U.S. Department of State issued a record 16,208,003 U.S. passports. Bloomfield Township processed 647 passport applications in 2008, compared to 1,283 in 2007. The Township first began offering passport service on August 15, 2005.

Passport fees are the same in each community. For persons 16 years old and older, there is a $110.00 fee payable by check or money order to the U.S. Department of State and a $25.00 execution fee payable by cash or check to Bloomfield Township. Passport fees for children 15 years old and under include a $80.00 fee payable by check or money order to the U.S. Department of State and a $25.00 execution fee payable by cash or check to Bloomfield Township. Passport card fees are $30.00 for an adult 16 years old and older and $15.00 for children 15 years old and under, in addition to a $25.00 execution fee payable to Bloomfield Township. The requirements and validity for a Passport Card are the same as for a U.S. Passport. Photo fees are only $10.00 per set. Photo fees are payable by cash or check to Bloomfield Township. Bloomfield Township Clerk’s Office offers complete, convenient passport service at 4200 Telegraph Road, Bloomfield Hills, MI 48302.NOTE: Valid passports are required for children of every age.

To apply for a U.S. passport, residents must provide:

  • A passport application form.
  • Proof of U.S. citizenship in the form of a previous passport, certified birth certificate or naturalization certificate.
  • Proof of identity, including a driver’s license, government or military ID Social Security cards will not be accepted.
  • Travelers who need additional information about new passport regulations may visit the U.S. Department of State web site or call the Bloomfield Township Clerk at 248.433.7702.

    Important news: Trips to Canada, Mexico, Bermuda, Central and South America will require passports by December 2006.Bloomfield Township Clerk’s Office offers complete, convenient passport service at 4200 Telegraph Road, Bloomfield Hills, MI 48302.

    Hours are 7:00 AM to 4:30 PM Monday through Thursday. Offices are closed on Friday.

    Photo service available for $10.00 per set.

    Important News: A convenient, economical, wallet-size Passport Card is available. However, it may only be used for land and sea travel to Canada, Mexico, the Caribbean, and Bermuda. This card cannot be used for air travel.

    Requirements

    BLOOMFIELD TOWNSHIP ACCEPTANCE AGENCY REQUIREMENTS FOR A U.S. PASSPORT/PASSPORT CARD

    1. Citizenship Documents – Submit one of the following:

    A. Previous U.S. Passport

    B. Certified copy of Birth Certificate – containing the following:

    1. Full birth name

    2. Date and place of birth

    3. File date, within one (1) year of birth

    4. Raised, embossed seal of certification and registrar’s signature

    C. Naturalization certificate (original)

    D. Report of birth abroad (FS-240, FS-545, DS-1350)

    E. If none of the above are available, contact the Passport Acceptance Facility for help

    2. Identification Documents – Submit one of the following:

    A. Valid driver’s license: no temporary licenses accepted

    B. Government photo ID (Federal, State, or Municipal)

    C. U.S. Passport (not expired)

    D. Naturalization/Citizenship Certificate (issued within the last 5 years)

    E. Valid Official Military Identification Card

    F. Current University Identification Card

    3. Passport Photographs -

    A. Two (2) duplicate recent photos (no more than six (6) months old)

    B. Full face

    C. 2" x 2" photo head size no smaller than 1" – maximum 1-3/8"

    D. Color preferred on light (white or off-white) background

    E. Photos available at the Bloomfield Township Clerk’s office $10.00 each set of two (2)

    5. All applicants 15 years old and under will require both parents or one parent and written notarized consent from second parent.

    All applicants 15 years old and under with a passport book must also submit a birth certificate as evidence of child’s relationship to parent.

    All applicants 17 years old with current driver’s license and check from parent may apply. Parent signature not required; however highly recommended for ID.

    6. You must have a COMPLETED application (Form DS – 11) DO NOT SIGN IT!

    7. ALL PERSONS APPLYING FOR A PASSPORT WITH FORM DS 11 MUST BE PRESENT!
